1. A completed quilt work order form should be included for each quilt shipped. (Click here for Quilt Work Order Form.) Also attach any special instructions or requests (return shipping preference, priority, insurance amount, etc.) to each quilt when shipping.
2. All items should be placed in a sealed box large enough for the completed quilt to be returned in. If your box is not big enough, additional charges will be assessedto accommodateyour quilts for returnAll items will be returned via UPS ground, unless otherwise specified by you on your order form.
3. A 50% deposit is required when quilting is requested. You can send a check or money order with your quilt or call ahead to charge to your Visa or MasterCard.
